Services At Trilogy: Linkage, Outreach and Triage ServicesTrilogy’s Linkage, Outreach and Triage Services (LOTS) program provides an array of services to adults with severe and persistent mental illness who present at the agency with an immediate need for service due to acute psychiatric decomposition or situational emergencies that require assistance with basic needs. LOTS Services Include: Crisis Services – The LOTS team administers a 24-hour psychiatric crisis line which is available to current clients as well as individuals and families in the community. Crisis Workers are available around-the-clock to provide face-to-face outreach, crisis assessment and intervention. Intake – Each month the Trilogy intake line receives dozens of calls from applicants, their friends or family members, and other community providers requesting mental health services. The Intake Coordinator responds to these calls quickly and efficiently to insure that applicants are linked with appropriate services at Trilogy or elsewhere. The Intake Coordinator also communicates regularly with other agencies and organizations in the community to form and maintain two-way referral relationships. Linkage Case Management – Many applicants for Trilogy services present with acute needs, such as homelessness, lack of access to medical and/or psychiatric care, or hunger. In such cases, Linkage Team Recovery Counselors are available to provide immediate case management to assist individuals in gaining greater psychiatric stability and access to community resources. The Linkage Team Recovery Counselors work closely with staff psychiatrists, and can schedule immediate appointments for those in need. Outreach Case Management – Trilogy’s Recovery Counselors are designated to provide case management intervention to individuals who are likely to “dropout” of treatment, because of symptoms of their illnesses. Recovery Counselors meet with clients in the community, and focus primarily on “starting where the client is”, i.e., engaging individuals in mental health treatment at a rate with which clients are most comfortable and intervening in ways the clients will find most concretely helpful.
