Trilogy’s approaches are trauma-informed and put the unique experiences of the individual at the center of treatment. Recovery and well-being are different for each of the individuals we serve. We work alongside every person, empowering them to not just participate in their recovery process but to define and guide it.

Our programs include:

Intake
Trilogy helps new and potential clients connect easily to behavioral and primary healthcare services across Chicago and beyond. Intake appointments are available by phone, and in-person by appointment. To inquire about available services at Trilogy, please call 773.382.4060.

Services are available in English and Spanish in most cases, and Medicaid is accepted.

Williams and Colbert Class Member Program
(Consent Decree Programs)

Trilogy’s Williams and Colbert Class Member Program supports people who live in nursing homes or rehabilitation facilities to transition into their own home. Trilogy partners with 39 nursing facilities across the region, working directly with interested residents to create customized transition plans. Each plan includes a needs assessment, support finding their new home, and ongoing wrap-around supports to ensure the transition is a success.

Front Door Diversion provides support and preventative interventions so that people experiencing a mental health crisis can maintain their independence in the community as opposed to being admitted to a nursing home or care facility.

Intensive Community Outreach Program (ACT and CST)
Our Intensive Community Outreach Program provides assessment and intervention, crisis services, and ongoing treatment for individuals who are experiencing a psychiatric crisis or need intensive outreach services and supports to remain stable in the community.

Clients are assessed for the level of care they need when they come through our Intake Program’s open access hours or are referred to the program, and individuals requiring crisis intervention receive stabilization support. Clients are linked to Assertive Community Treatment (ACT, the highest level of community outreach support), a Community Support Team (CST) or other level of care as needed. The majority of services take place in community settings to best support clients. Staff facilitate appropriate treatment using a shared caseload model that supports clients to work on their individual treatment goals and move toward stability. Clients receive home visits, assistance with appointments, linkage to resources and benefits in the community, support with symptom management and building coping skills, and ongoing wrap-around services.   

In addition, Intensive Community Outreach supports individuals who are transitioning to the community from institutional settings such as nursing homes, hospitals, jails, and prisons.

Recovery Services
Recovery Services provides support to people who require case management and support to live independently. Clients work with their Recovery Counselor to identify personal recovery goals and create a roadmap to achieve them. Depending on their goals, clients may receive support with managing medication, securing benefits, grocery shopping, and more. In addition, the Recovery Services team advocates on behalf of clients when needed.

Integrated Health Clinic
Trilogy provides comprehensive healthcare services for our clients, enabling us to customize care to best meet the needs of every person we serve. The Trilogy Heartland Integrated Health Clinic, operated in coordination with our partners Tapestry 360 Health and Genoa Pharmacy, provides accessible, well-coordinated care for both psychiatric and general health conditions. Services include psychiatry evaluations and medication management, primary care, health promotion and care coordination, and are provided by physicians, nurse practitioners, pharmacists, nurses, medical assistants, phlebotomists, and support staff, all working in collaboration with Trilogy teams. By providing an array of healthcare services in one location, Trilogy provides access and convenience for clients who have complex mental health and medical conditions, and supports clients to reach their goals by providing a comprehensive healthcare service home.

InSHAPE
is a comprehensive wellness program that improves the health and longevity of people experiencing mental illness. Clients receive fitness coaching with a certified personal trainer, as well as assistance with meal planning, nutrition, and cooking guidance. As with all Trilogy programs, clients take responsibility for setting their own goals and directing the desired outcomes.

Outpatient Services
Trilogy’s Outpatient Services offer a wide range of educational, social and psychotherapeutic groups to help clients better understand and manage their illnesses, develop effective coping skills, build vocational skills and improve social functioning. Individual, group and family therapy available for adults and youth in several Chicago locations. These include:

• Cognitive Behavioral Therapy (CBT),
a skills-based, problem-focused treatment intervention that supports clients to identify and modify negative patterns of thinking and respond to situations more effectively. CBT has been shown to be effective in the treatment of anxiety, obsessive-compulsive disorder, post-traumatic stress disorder and depression.

• Dialectical Behavioral Therapy (DBT),
a form of CBT, helps clients manage intense feelings and emotions. DBT approaches support clients to expand the range of responses available to them in the face of difficult or painful feelings. There are four primary modules of DBT:
core mindfulness, interpersonal effectiveness, distress tolerance and emotion regulation. These modules are explored with a systematic and gradual process.

• Hope First
For those who have recently experienced a first episode of psychosis, Hope First provides interventions to reduce the symptoms of psychotic illnesses, improve client and family functioning, promote recovery and reduce the chance of relapse

• Eye Movement Desensitization and Reprocessing (EMDR)
is a therapeutic approach that helps clients re-process traumatic memories in a way that is less disturbing and disruptive to their lives.

• Trauma Therapy helps clients address the initial shock and crisis of a traumatic event, process their trauma, and move towards life balance.

• Couples and Family Therapy
When families and couples are involved in their mental health treatment together, they provide an additional level of support for one another. Couples and Family Therapy provides a supportive atmosphere to strengthen partnerships and develop problem-solving and coping skills that enhance communication and promote healthier relationships. Family therapy can involve the whole family, or those interested in participating. Couples therapy must include both individuals in the relationship.

• Child and Adolescent Therapy, which addresses the specific needs of youth ages 6 and up, provides a focus on ensuring long-term success. Medicaid accepted.

• Integrated Dual Diagnosis Treatment (IDDT) supports people with mental illness and substance use issues.

• Our Latino Services program increases access to linguistically and culturally responsive recovery-oriented services, filling a critical service need for Latinos living with serious mental illness.

Occupational Therapy
Our Occupational Therapy program supports clients to strengthen their abilities to carry out a variety of activities that promote independence, wellbeing, and promote overall quality of life. Occupational Therapy (OT) can support a client to improve their ability to accomplish activities of daily living (eating, bathing, grooming) and instrumental activities of daily living (grocery shopping, meal preparation, laundry, and more), safely and with increasing levels of independence. OT can also support clients to discover and engage in activities that are meaningful to them, including identifying interests, engaging in community activities, increasing social skills, and more. Interventions might include skill-building, identifying adaptive equipment and transportation options. Trilogy’s Occupational Therapists collaborate with clients to determine what activities are meaningful to them, and then develop plans to support goal attainment.

Trilogy’s nationally-recognized Supported Employment Program assists individuals with mental illness or substance use disorders to find and retain competitive jobs at top local and national employers based on their skills, abilities, and preferences.

Our Supported Employment Program (SEP) uses an evidence-based practice known as Individual Placement & Support (IPS) model to assist and support individuals with mental illness and substance use disorders to achieve and maintain competitive, preference-based employment. The Employment Specialists support the client through all five phases of the IPS model, including: Engagement, Assessment, Rapid Job Search/ Job Development, Job Retention Supports, & Job Termination Assistance. Employment Specialists (ES) identify and build relationships with community-based employers and worksites to develop job placements and settings identified by the clients. There are not requirements or disqualifications for a client to join the SEP program, as anyone who says they want to work can join the program. The model is guided by eight principles including: advocacy for an individual’s desire to work; supporting personal preferences; providing time unlimited job supports; and close integration with an individual’s mental health team members. Employment is a powerful tool in one’s recovery, as it can foster personal growth in multiple facets of a person’s life.

Residential Programs
Stable housing is a fundamental component of recovery. Trilogy’s Residential Programs provide comprehensive housing options, from community-based supported housing to 24-hour residential support.

The Beacon, Trilogy’s drop-in center, provides activities that build skills, increase community involvement, and provide socialization opportunities. Activities include support building coping and problem-solving skills, symptom self-management, and more. The Beacon is entirely peer-led, meaning that staff have lived experience with mental illness.

PATH, or Projects for Assistance in Transition from Homelessness, connects people experiencing homelessness with services that help them move toward recovery, such as behavioral and general practice healthcare, substance use counseling, and permanent housing placement.

First-response Alternative Crisis Team (FACT)
Trilogy’s First-response Alternative Crisis Team (FACT) provides urgent mobile response and interventions, as well as follow-up support, for individuals who are experiencing a mental health crisis. FACT is designed to provide mental health interventions in situations that are not immediately life-threatening that require urgent support from trained mental health professionals. FACT is currently operating in Chicago’s Rogers Park, Edgewater, West Ridge, and Uptown neighborhoods, as well as Skokie and Evanston.
